Registration

Club Membership Application*
Clubs are required to register their club annually by June 1 of each year. Required Documents:
Club Membership Application, $100.00 check, copy of current club bylaws and policies (electronic copy),
Club contact list (updated online in Affinity), copy of the club's most recent form 990 or 990EZ and \
CT-12 (electronic copy), a checklist is included in the form. Electronic copies may be sent as email
\attachments to: brian@oregonyouthsoccer.org.

Age chart to determine, by birthday, the age which a player is eligible to play.

Washington State Concussion Awareness Law and Form

Washington State has passed a law requiring annual informed consent forms to be signed by all
parents and players regarding the signs and dangers of concussions.  This form must be carried
by the team during any play that occurs in Washington State.

Eighth Grade Waiver*
For all recreational and competitive U14 teams - any U15 player still in the 8th grade is eligible to
play on the U14 team when the Eighth Grade Waiver form is properly completed, submitted to Oregon
Youth Soccer Association and approved by the State Registrar.  These players can not play in the
Presidents or State Cup with the U14 team.  They must be unrosteed to any team particiapting in Cup play.

Background Disclosures
Background disclosures may only be submitted as part of registration through OYSA's online registration
system.  If you are a coach or other club administrator, contact your club registrar for the URL to your
club's registration page and submit your disclosure as part of registering for the club.  You will need to
complete an application for each season your team is entering into (Fall and Spring).  If you already are
registered in the database as a parent, your club registrar can assist you in creating an Administrator's
registration.  If you need any other assitance in completing your background check, contact Desia Bramel

District List

USYS 2010 Interstate Release Form
Out of state players need to register in the state in which they reside prior to registering in Oregon.
This applies to players both Recreational and Competitive.  The paperwork could take 2 to 3 weeks
to process.  A player can not be rostered in Oregon until we receive the release agreement from their
resident state, nor should the player be training with the Oregon team until the release is complete.

These forms are available to anyone who needs a medical release for club or tournament use.  Two
samples are provided above.  One has a requirement for a Notory stamp/signature, the other does not.  It
is up to the Club which form they require.

International Clearance Process
Any player born outside of the United States must receive clearance from the USSF before being
registered to play in the United States.  Click on the link to view the Federation's explanation of the
clearance process and to obtain any necessary forms.
