Administration Forms

Clubs are required to register their club annually by June 1st. Forms Needed: Club Membership Application, Roster of Club officers, directors and important contacts, $100.00 check, copy of current club bylaws and policies, copy of the club's 501(c)(3) determination letter, and a copy of the club's most recent form 990 or 990EZ. A checklist is included in the form.

For Fall competitive U14 teams only, any U15 players still in the 8th grade are eligible to play on a U14 team when the Eighth Grade Certification/Waiver form, properly completed, is submitted to Oregon Youth Soccer Association with that player's team registration. These players cannot play in the Presidents or State Cup with the U14 team.

Background disclosures may only be submitted as part of registration through OYSA's online registration system. If you are a coach or other club administrator, contact your club registrar for the URL to your club's registration page and submit your disclosure as part of registering for the club. If you already are registered in the database as a parent, the club registrar can assist you in creating an Administrator's registration. If you need to complete a background check for any other purpose within OYSA, click here to submit a disclosure.

Out of state players need to register in their own state prior to registering in Oregon. This applies to players for Recreational and Competitive teams. The paper work could take 2 to 3 weeks to process. A player cannot be rostered in Oregon until we receive the release agreement from their residence state, nor should the player be training with the Oregon team until the release is complete.

This form is available to anyone who needs a medical release for club or tournament use.
